Terms of Service

Payment & Refund Policy

At Ideal Mental Health and Wellness Care, we value your time and ours. Appointments canceled with less than 24 hours’ notice will be charged a $100.00 cancellation fee. Payments for psychiatric evaluations and consultations are non-refundable once the service has been rendered.

Telehealth Informed Consent

By using the services provided by Ideal Mental Health and Wellness Care, you acknowledge and agree to the following:

  • Nature of Telehealth: I understand that “telehealth” includes the practice of health care delivery, diagnosis, consultation, treatment, and transfer of medical data using interactive audio, video, or data communications.
  • Benefits & Risks: I understand that telehealth allows for convenient access to care, but it also carries risks, including potential technical interruptions or unauthorized access. In the event of a technical failure, my provider will contact me via the phone number provided.
  • Verification of Location: I agree to provide my current physical location at the start of every session. I understand that my provider is only licensed to treat me while I am physically located in Florida, New York, Washington, or Oregon.
  • Emergency Limitations: I understand that telehealth is not for emergencies. In a crisis, I will call 911 or go to the nearest emergency room.
  • Prescription Policy: I understand that a telehealth evaluation does not guarantee a prescription. The decision to prescribe medication is at the sole clinical discretion of the provider, following all state and federal regulations (including the Ryan Haight Act).
